If you meet any of the eligibility requirements below, you must fax in your request for a Vote by Mail ballot no later than 5 pm on Tuesday, February 26.

Once you fill out this form, we will insert your information into the Vote by Mail ballot request and email you back a completed form. All you have to do is download, print, sign, and fax to your early vote precinct. Find out where you are registered to vote.
